We started this venture because we saw that hospice can be done in a more loving, dignified, personalized setting. Our first venture, Hearthstone Communities, has grown to four home care facilities staffed with our amazing professionals. As our network grew we saw a need with patients needing hospice and Hearthstone Hospice was born.

We are a team of four health care professionals that started a care home because we didn't like what health care had to offer in our area. Our first venture, HearthStone Communities, has grown to four home care facilities staffed with our amazing professionals.

Our Hospice care is an interdisciplinary approach to holistic care designed to meet the needs of a person at the end of life when cure is no longer an option. Hospice considers the unit of care to extend beyond the patient and include the family and caregivers as defined by the patient.

 

Our Mission is to provide optimal comfort and dignity to our patients through their end of life journey.  We will achieve this by concentrating on patient and family centered goals with a focus not only on the medical needs but also by providing care for the mental and spiritual needs of our patients and families during their journey.

Medical Director

Overseas each patient‘s plan of care, advises the hospice team and is available 24 hours a day, 7 days a week.

Registered Nurse

Supports the patient thought the process of end-of-life care by providing direct care, managing pain and other symptoms as they arise and providing support, comfort and education to families caring for their loved ones.

 

Certified Nurse Assistant

Supports the patient and family by assisting with with personal care, including bathing, dressing, toileting and incontinence care, light meals or housekeepin, spending time with patients and families, and communicating changes to their hospice nurse case manager.

 

Medical Social Worker

Helps the patient and family cope with emotional, social, physical and financial stress, as well as provides counseling as need it.

 

Spiritual Care Coordinator

Offers spiritual support to the patient if desired, and serves as a liaison between patient/family and local clergy of their choice.

 

Volunteers

Specially trained community members who provide non-medical support and assistance to patients and their families including respite care, companionship and help with errands.

 

Bereavement Coordinator

Arranges ongoing bereavement services, support and education for grieving family and friends.
